What is Forms 1094c and 1095c?

Form 1094 C and Form 1095 C are the forms required for the information required for the healthcare in the IRS. According to the Affordable Care Act, all the applicable large employers (ALEs) are required to report whether every employee has been offered proper health coverage or not. Form 1094 C contains the necessary information about the employees. The employers must file form 1094 C and 1095 C for the employees who have been working in the month of the calendar year.

The formula for 1094 C and 1095 C:

Full-Time Employee Count + (Total Part-Time Hours/30) = FTE Total

For an employer to be considered an Applicable Large Employer, the company must employ more than 50 full-time employees.
